Unique Device Identification Declaration

GS1 is an accredited issuing agency for the US Food and Drug Administration Unique Device Identification program. This program allows all items sold in the US that are classified as medical devices to be uniquely identified in all business systems used to handle or track those products.

As part of our obligations, we are required to report on the currency of suppliers who have products covered under this program.

To enable this we need to confirm if your business manufactures products that are considered medical devices and are sold in the US.
